Cheif complaints A 29year old female came to casuality c/o general weakness from 4days 
Fever from 4days 
Hematuria from today afternoon
History of present illness
Pt was apparently asymptomatic 5days back she developed fever since 4days 
Which is continuous high grade fever assosiated with chills and rigour now sub sided after taking treatment at outside private hospital investigations showing 92000 platelet counts 
Nw c/o hematuria blood in urine after passing urine from afternoon 
General weakness from 4days
No h/o burning micturition
 No history of loose motions
No h/o other bleeding diathesis like bleeding from gums
Malena
Past history not significant no comorbidities
Family history not significant 
Personal history appetite normal
Diet mixed diet
Sleep normal
Addictions nil
Micturition blood in urine from today afternoon
Bowel movements regular
General examination
O/o pt c/c/c
No pallor,icterus,cyanosis, clubbing, lymphedenopathy,koilonychia,pedal edema
Temp 97.2 f
PR 82bpm
Bp110/80mm of hg
